About Letscares
Professional expertise guided by compassion, dignity and a commitment to meaningful care.
Letscares was founded by Chantelle Eastman, a qualified healthcare social worker dedicated to bridging the gap between healthcare professionals, patients, families and community resources.
Social Work & Care Coordination
Compassionate support. Professional guidance. A clearer way forward.
At Let's Cares, we believe that every person deserves to be treated with dignity, compassion and respect — especially during life's most challenging moments.
Founded by Chantelle Eastman, a social work professional with experience in healthcare and psychosocial support, Let's Cares provides personalised social work services and care coordination to individuals, families and older adults navigating complex healthcare needs, changing circumstances and important life decisions.
We understand that a hospital admission, a new diagnosis, a dementia journey or the changing needs of an ageing loved one can feel overwhelming. Knowing where to turn, which services to access and how to coordinate the right support can be difficult for families already carrying so much.
Our role is to bring clarity, structure and compassionate guidance to these situations.
Through a person-centred and holistic approach, we assist clients and their families in understanding their needs, exploring appropriate care options, coordinating support services and planning for safe, dignified and sustainable care.
Our services may include psychosocial assessments, hospital discharge and care planning, dementia-related support, family guidance, care coordination, resource navigation and assistance with accessing appropriate community and healthcare services.
At Let's Cares, we recognise that no two individuals or families are the same. We take the time to listen, understand each person's circumstances and develop practical, individualised support plans that respect personal choices, preserve independence where possible and promote quality of life.
We believe that effective care extends beyond a diagnosis or a hospital stay. It involves understanding the person, supporting the family and connecting the right people and services to create a more manageable way forward.
Because caring is not simply about providing assistance. It is about helping people feel heard, supported and confident in the decisions that affect their lives.
Let's Cares — compassionate support, thoughtfully coordinated.
