About the Services

Let's Cares offers professional social work services and personalised care coordination to individuals, families and older adults navigating healthcare challenges, hospital admissions, rehabilitation, dementia-related needs and significant life transitions.

Services may include psychosocial assessments, discharge planning, care coordination, family consultations, guidance on care options, referrals to appropriate service providers and ongoing support, depending on the individual's needs.

Our services may be suitable for older adults who need additional support to maintain their independence; individuals living with dementia or other changing healthcare needs; patients preparing for discharge from hospital or rehabilitation; families coordinating care across different cities or countries; adult children concerned about an ageing parent; families navigating difficult care decisions; and healthcare professionals seeking social work support or care coordination for a patient.

Every referral is considered individually, with the aim of identifying appropriate, practical and person-centred support.

Yes. Let's Cares can assist individuals and families affected by dementia through care planning, family guidance, resource navigation and coordination with relevant healthcare and support providers.

This may include helping families understand changing care needs, consider appropriate support arrangements, prepare for transitions and coordinate communication between those involved in care.

The focus is on dignity, person-centred support, quality of life and practical guidance for families.

Home visits may be arranged where clinically or practically appropriate and subject to location, availability and the nature of the service required.

A home visit can help clarify practical support needs, environmental considerations and the arrangements required to assist someone safely at home.

Travel fees, if applicable, will be discussed before the appointment is confirmed.

Yes. Care coordination often involves communication between several people and services.

With appropriate consent and within professional boundaries, Let's Cares can liaise with relevant healthcare professionals, facilities, caregivers and family members to support coordinated planning and continuity of care.

Professional collaboration is intended to complement, rather than replace, the responsibilities of the treating team.

Depending on the referral, professional scope and information available, Let's Cares may provide psychosocial assessments, care-related reports and written recommendations.

The purpose, intended recipient, information required, assessment process and applicable fees will be clarified beforehand. Reports will reflect the assessment undertaken and the evidence available.

Any formal legal, statutory or specialist assessment will be subject to the relevant requirements and the professional's authority to undertake it.

We can assist families in identifying and exploring relevant care options and support services based on their needs, location, preferences and budget.

Depending on the circumstances, this may include home-care services, residential care facilities, rehabilitation providers and other relevant healthcare professionals.

Availability, suitability, fees and credentials should always be confirmed directly with the relevant provider. A referral or introduction does not constitute a guarantee of a provider's services.

Families & Care Decisions

Absolutely. Living abroad can make it difficult to coordinate care for a parent, relative or loved one in South Africa. Let's Cares can assist with communication, care planning, coordination with relevant service providers and guidance on available local support.

Consultations and family meetings can be arranged online, allowing you to participate in important discussions and care decisions from wherever you are in the world.

With the appropriate consent and permissions, we can liaise with relevant family members, healthcare professionals and care providers to help keep everyone informed and the care plan coordinated.

Yes. Early involvement can help families prepare for the transition from hospital to home or another care setting.

Depending on the circumstances, assistance may include identifying potential care needs, discussing available support options, coordinating with relevant professionals and helping families plan for practical arrangements before discharge.

The aim is to support a more organised transition and reduce avoidable uncertainty for the patient and family.

Yes. We can help families explore care needs, available support, environmental considerations and possible care arrangements.

Options may include additional home-based assistance, caregiver support, rehabilitation, community services or consideration of a residential care setting where appropriate.

Recommendations depend on an individualised assessment and the information available. The final decision should take account of the person's wishes, safety, care requirements and available resources.

Care decisions can become difficult when family members have different views, particularly when a loved one's needs are changing.

Where appropriate, Let's Cares can facilitate structured discussions, clarify care-related concerns, identify practical options and help families focus on the person's needs, wishes and best interests.

This service does not replace legal advice, formal mediation or court processes where those are necessary.

You do not need to have all the answers before contacting us.

Many families reach out because they feel overwhelmed, are unsure which services to approach or need help understanding their options. An initial discussion can help clarify the concerns, identify possible next steps and determine whether Let's Cares is the appropriate service for your situation.

Getting Started & Referrals

The first step is an initial discussion to understand your concerns, the individual's circumstances and the assistance you are seeking.

Depending on the referral, the process may involve an initial telephone or online consultation; clarification of the presenting needs and relevant background; agreement on the scope of the service and applicable fees; an assessment or consultation, where appropriate; a proposed care plan, recommendations or coordination of agreed services; and follow-up, if required and agreed.

Each situation is different, so the process will be tailored to the person's needs.

Referrals are welcome from family members, hospitals, medical practitioners, allied healthcare professionals, attorneys, residential care facilities and other relevant service providers.

You can contact Let's Cares by email, telephone or WhatsApp to discuss the person's circumstances and the support required.

With the appropriate consent, we will establish the nature of the referral, clarify the individual's needs and explain the recommended next steps, including any assessment, consultation or care coordination that may be appropriate.

Professional referrals can include a brief summary of the presenting concerns, relevant care needs and the referring professional's contact details. Please avoid sending unnecessary confidential medical information before a suitable communication channel has been established.

You may contact us to discuss your concerns and obtain general guidance about possible support options. However, any assessment, intervention or sharing of personal information involving the individual will need to respect their rights, consent, confidentiality and decision-making capacity, as applicable.

Where a person cannot make certain decisions independently, the appropriate legal and ethical processes will need to be considered.

Yes. Depending on the nature of the service, consultations can take place through online meetings or telephone calls. WhatsApp may be used for arranging appointments, sharing appropriate updates and maintaining communication.

Online consultations are particularly useful for families living in different cities or countries, as well as those who cannot easily attend an in-person meeting.

Please note that WhatsApp is not an emergency service, and sensitive information should be shared through appropriate, secure channels.

Response times depend on availability, the nature of the referral and the urgency of the situation.

Where possible, please indicate if a hospital discharge, change in care arrangements or another time-sensitive matter is approaching. This helps us establish whether assistance can be offered within the required timeframe.

Let's Cares is not an emergency response service. Immediate medical emergencies should be directed to the appropriate emergency services.
